Overview
The ulnar collateral ligament (UCL) stabilizes the inner elbow and is the classic overhead-throwing injury. Sprains and tears cause inner-elbow pain and loss of throwing performance, most often in baseball players.
Common Symptoms
- Inner elbow pain when throwing
- Loss of velocity or control
- A 'pop' at the time of injury
- Tingling into the ring and small fingers
Causes & Risk Factors
- Repetitive overhead throwing
- High pitch counts and poor mechanics
- A single high-stress throw
Treatment Options
- Rest and a progressive throwing program
- PRP for select partial tears
- UCL reconstruction ('Tommy John') for complete tears
Recovery & What to Expect
Partial tears may respond to rest and rehabilitation. Reconstruction has a high return-to-play rate, typically over 9–12 months for throwing athletes.
