Shoulder Impingement & Bursitis

Overview

Shoulder impingement occurs when the rotator cuff tendons and the overlying bursa are pinched beneath the acromion, producing pain with overhead reaching. It is one of the most common causes of shoulder pain and usually responds well to therapy that restores mechanics and reduces inflammation.

Common Symptoms

  • Pain reaching overhead or behind the back
  • Pain at night, especially lying on the shoulder
  • A painful arc of motion
  • Aching over the outer shoulder

Causes & Risk Factors

  • Repetitive overhead activity
  • Weak or imbalanced rotator cuff and scapular muscles
  • Bone spurs narrowing the space
  • Poor posture and mechanics

Treatment Options

  • Physical therapy for cuff and scapular strength
  • Activity modification and anti-inflammatories
  • Image-guided subacromial injection
  • Arthroscopic subacromial decompression for persistent cases

Recovery & What to Expect

The large majority improve with a structured therapy program over 6-12 weeks. The small number with persistent symptoms may benefit from a minimally invasive arthroscopic procedure.
