Patellar Tendon Tear

Diagnosis & treatment by Kevin O'Donnell, MD — Coral Gables, FL

Overview

The patellar tendon connects the kneecap to the shin and is essential for straightening the knee. A complete tear, typically in younger, active patients from a forceful jump or landing, disrupts the extensor mechanism and requires surgical repair to restore function.

Common Symptoms

  • A pop and sharp pain below the kneecap
  • Inability to straighten the knee
  • A kneecap that sits abnormally high
  • Swelling and a gap below the kneecap

Causes & Risk Factors

  • A forceful jump or landing
  • A fall onto a bent knee
  • Prior patellar tendinitis or weakening

Treatment Options

  • Prompt surgical repair of the tendon to the kneecap
  • A protected brace-and-rehabilitation program
  • Physical therapy to restore motion and strength
  • Gradual return-to-sport progression

Recovery & What to Expect

Repaired tendons are protected early and progressed through a structured program, with return to full activity and sport over 4-6 months.
