Patellar Instability

Diagnosis & treatment by Kevin O'Donnell, MD — Coral Gables, FL

Overview

Patellar instability is recurrent dislocation or slipping of the kneecap, often from ligament injury or underlying alignment issues. It is common in adolescents and young athletes.

Common Symptoms

  • The kneecap slipping or dislocating
  • Pain and swelling after an episode
  • Apprehension with bending
  • A feeling of the knee giving way

Causes & Risk Factors

  • A twisting injury or direct blow
  • Loose or torn stabilizing ligament (MPFL)
  • Alignment and anatomy that predispose to dislocation

Treatment Options

  • Bracing and a strengthening program
  • MPFL reconstruction
  • Bony realignment for select cases

Recovery & What to Expect

First-time dislocations may be managed with therapy; recurrent instability is treated surgically, with return to sport typically at 4–6 months.
