Olecranon Fracture

Diagnosis & treatment by Kevin O'Donnell, MD — Coral Gables, FL

Overview

The olecranon is the bony tip of the elbow where the triceps attaches. A fracture, usually from a direct fall onto the elbow, often displaces because the triceps pulls the fragment away, and typically requires fixation to restore the ability to straighten the elbow.

Common Symptoms

  • Pain and swelling at the tip of the elbow
  • Inability to straighten the elbow against gravity
  • A palpable gap at the elbow tip
  • Bruising and tenderness

Causes & Risk Factors

  • A direct fall onto the elbow
  • A direct blow
  • High-energy trauma

Treatment Options

  • Splinting for rare non-displaced fractures
  • Tension-band or plate fixation for displaced fractures
  • Early protected motion to prevent stiffness
  • Physical therapy to restore motion and strength

Recovery & What to Expect

Fixed fractures begin early protected motion, healing over about 6-12 weeks. Hardware occasionally causes irritation and is sometimes removed later; most patients regain good function with therapy.
