Distal Humerus Fracture

Diagnosis & treatment by Kevin O'Donnell, MD — Coral Gables, FL

Overview

A distal humerus fracture is a break of the lower end of the arm bone at the elbow. These are often complex, involving the joint surface, and typically require surgical fixation to restore alignment and allow the early motion that is essential to avoid a stiff elbow.

Common Symptoms

  • Severe pain and swelling around the elbow
  • Deformity and bruising
  • Inability to move or straighten the elbow
  • Tenderness over the bone

Causes & Risk Factors

  • A fall onto the elbow or outstretched arm
  • High-energy trauma
  • Weakened bone in older adults

Treatment Options

  • Splinting and early protected motion when stable
  • Open reduction and internal fixation for displaced fractures
  • Structured therapy to restore elbow motion
  • Elbow replacement for select severe fractures in older patients

Recovery & What to Expect

Stable fixation allows early motion to prevent stiffness. Bone healing takes about 6-12 weeks, with strengthening and full return to activity over several months.
