Cubital Tunnel Syndrome

Diagnosis & treatment by Kevin O'Donnell, MD — Coral Gables, FL

Overview

Cubital tunnel syndrome is compression of the ulnar nerve as it passes behind the inner elbow, the 'funny bone' nerve. It causes numbness and tingling in the ring and small fingers and, over time, hand weakness. Symptoms are often worse with the elbow bent, such as holding a phone or sleeping.

Common Symptoms

  • Numbness and tingling in the ring and small fingers
  • Symptoms worse with the elbow bent
  • Weak grip and pinch, clumsiness
  • Aching at the inner elbow

Causes & Risk Factors

  • Prolonged or repetitive elbow bending
  • Leaning on the elbow
  • Prior elbow injury or arthritis
  • Nerve instability at the elbow

Treatment Options

  • Activity modification and a night elbow splint
  • Padding and ergonomic changes
  • Nerve-gliding physical therapy
  • Cubital tunnel release, with or without nerve transposition

Recovery & What to Expect

Mild cases often improve with splinting and activity changes. When surgery is needed, numbness typically improves first, with strength recovering over weeks to months depending on severity.
